Customer Application Form
Thank you for your interest in working with ASHBROOK. Download our New Customer Account Application Form, via the link below.
The form can be filled in electronically. Alternatively, please print the form (PAGE ONE and TWO of the seven-page document). In both cases, fill it in, sign it, and send it back to us. Please include the following supporting documents:
- A company letterhead including the company number and VAT number (if applicable)
- A copy of a recent utility bill showing proof of address
- A copy of a current Photo ID e.g. driving license or passport
- Proof of insurance to cover the hired in plant

Please note: to help expedite the application process ASHBROOK accept clear, legible photographs of the application form and the supporting documents sent via email to your most convenient depot:

We operate under the ASHBROOK Terms and Conditions which are included in the form and for plant hire these incorporate the CPA Model Conditions for the hiring of plant (2011).
